What does a clinical research site do?

Inside the organisations that actually run clinical trials and see participants.

Published by Clinrol

A clinical research site is the place where a clinical trial actually happens: where participants are screened, consented, dosed, examined and followed up. Sites take many forms, including dedicated research clinics, hospital departments, university units, GP practices that run studies alongside routine care, and networks that operate multiple locations under one organisation.

Every site is led by a principal investigator, a qualified clinician who is responsible for the conduct of the trial and the safety of participants at that location. Around the investigator works a team of study coordinators, research nurses, pharmacists and administrative staff who manage the protocol's day-to-day demands: scheduling visits, collecting data, dispensing study treatment, reporting adverse events and hosting monitoring visits from the sponsor or CRO.

Sites earn revenue through per-participant payments negotiated in clinical trial agreements. This makes enrolment the engine of the entire business model. A site that recruits well gets selected for more studies, negotiates better terms and grows; a site that repeatedly under-enrols gets passed over, however excellent its clinical care. Sponsors track site enrolment performance closely and historical performance heavily influences site selection.

That is why recruitment is the defining operational challenge for most sites. Coordinators juggle patient outreach between clinical duties, enquiries arrive around the clock while phones are only answered in office hours, and every no-show wastes an appointment slot the site cannot bill. Studies routinely show a large share of site-received referrals never convert because follow-up came too late.

High-performing sites treat recruitment as a discipline: they respond to enquiries within minutes, pre-screen against the protocol before booking visits, maintain and re-engage a consented patient database, and track their funnel per study so problems surface early.

Purpose-built technology makes that discipline sustainable. Clinrol Connect gives sites AI-assisted calls and SMS, protocol-specific pre-screening, automated booking and reminders, and recruitment analytics in one platform, so the site's enrolment numbers stop depending on how many hours coordinators can spend on the phone.
