What is a CRO and how do CROs work with sponsors?

Contract research organisations explained: what they do and how they fit into trials.

Published by Clinrol

A contract research organisation, or CRO, is a company that provides outsourced research services to the sponsors of clinical trials, typically pharmaceutical, biotech and medical device companies. Rather than building every capability in-house, sponsors contract CROs to run some or all of the operational work involved in taking a treatment through clinical development.

The services CROs provide span the whole trial lifecycle: study design and protocol development, regulatory submissions, site selection and management, clinical monitoring, data management and biostatistics, pharmacovigilance, medical writing and project management. Some CROs are full-service global organisations running trials across dozens of countries, while others specialise in a region, a therapeutic area or a specific function.

The sponsor remains legally responsible for the trial, but delegates defined responsibilities to the CRO through a contract and a formal transfer of obligations. In practice the CRO becomes the operational engine of the study: its project managers coordinate timelines and budgets, its clinical research associates monitor sites for data quality and protocol compliance, and its teams handle the day-to-day communication with research sites.

For research sites and patients, the CRO is often the face of the study. Sites are usually selected, trained and monitored by CRO staff, and study questions typically route through the CRO rather than the sponsor directly.

Patient recruitment sits in an interesting gap in this model. CROs manage sites and monitor enrolment, but generating patient interest and pre-screening candidates is a specialist marketing and screening capability that many CROs subcontract. That is why sponsors and CROs often bring in a dedicated recruitment partner when enrolment projections look challenging, and why recruitment accountability should be agreed early rather than after the enrolment curve flattens.

Clinrol works alongside both sponsors and CROs in exactly that role, supplying pre-screened, verified patients to their sites while the CRO runs study operations. If you are planning a study and recruitment risk is on your register, it pays to scope that support at the same time as you scope the CRO contract.
